OhioRISE Care Coordinator
Wayne and Holmes Counties

 

Shape the Future of Ohio Youth as an OhioRISE Care Coordinator with Coleman Health Services! Are you ready to be part of a visionary team that empowers Ohio's youth to thrive? Your passion for making a difference is what we're looking for! Join us as an OhioRISE Care Coordinator and harness the power of high-fidelity wraparound principles to drive positive change in the lives of youth and families within our community. This is a full-time position that will be serving Wayne and Holmes Counties in Ohio.

Your Impact as a Care Coordinator:

  • Conduct comprehensive behavioral health and psychosocial assessments.
  • Deliver high-fidelity wrap-around services to empower multi-system youth and their families.
  • Lead child and family team meetings, fostering collaboration and positive outcomes for youth living with mental health and/or addiction issues.
  • Create and execute strength-based child and family care plans, tailored to their unique needs.
  • Facilitate referrals and connect families with vital services and supports.
  • Maintain meticulous and timely documentation in an Electronic Health Record.

Qualifications of a Successful Care Coordinator:

  • High school diploma or equivalent with 3 years of experience in children's behavioral health or a related public sector; or 2 years experience with an associate's/bachelor's degree; or 1-year experience with a master's degree
  • Collaborative spirit, excelling in teamwork with youth, families, community providers, interdisciplinary teams, and child-serving systems.
  • Adaptability and resourcefulness to handle complex cases, finding solutions amidst rapidly changing situations.
  • Skillful facilitation of Child and Family Team Meetings, guiding diverse individuals toward shared goals.
  • Ability to obtain NPI and Ohio Medicaid numbers.
  • Maintain an outstanding driving record.
  • Prefer candidates with a BA/BS in social work or related field.
  • Prefer candidates with LSW, LPC, or higher.
