Care Coordinator (LSW)
Portage County, OH

 

Coleman Health Services, formerly Coleman Professional Services (CPS), is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community.  Due to our proven record of success, we recently earned a nearly $4 million Certified Community Behavioral Health Clinic (CCBHC) grant to provide a comprehensive range of mental health and substance use disorder services to vulnerable individuals.

Our new Care Coordinator position has the important role of helping our clients get the care they need by providing high-level coordination and collaboration with primary and specialty care providers in Portage County, Ohio.  In this role, you'll perform assessments, establish treatment plans, monitor progress, assess client reports of medical co-morbid disorders, promote client health, monitor caseloads and delegate/assign care tasks, educate clients and families, identify and overcome barriers to care, and more. 

Candidates must have an LSW, valid NPI number, and Ohio Medicaid number and at least two years of social work experience and with documenting in electronic health records.  Due to the teamwork required, candidates must be able to work effectively and collaboratively as a team, interacting with clients, external care providers and stakeholders, clinicians, and others in a diverse workforce while being able to exercise balanced judgement in evaluating situations and making decisions, and handling difficult and confrontational situations in a calm, consistent, and equitable manner.  An acceptable driving record with a willingness to transport clients is needed as well.  Additionally, candidates must be willing to work evenings, weekends, and holidays as needed to meet program needs and participate in on-call rotation.
