Case Manager (Mobile Response)
Coleman Health Services, formerly Coleman Professional Services (CPS), is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking case managers who will work in collaboration with Mobile Crisis Counselors to de-escalate and stabilize youth in crisis and their families and to provide short term wraparound services in Portage County, Ohio. The MRSS Case Manager assists clients with accessing community based resources, identifying supports and building skills to stabilize the crisis and prevent future crises from occurring.
- Provide short-term, community-based case management to youth and their families
- Assist with a warm handoff to long-term behavioral health providers and supports
- Work collaboratively with other members of the MRSS team to further support youth and families throughout the stabilization and transition phases.
- Function as part of an interdisciplinary team and develop and maintain effective relationship with community professionals
Qualified candidates will have their Bachelor’s degree in a related field or three years related experience (includes internships and volunteer experience); have and maintain an excellent driving record; and familiar with trauma informed care, community resources, motivational interviewing, crisis intervention, and case management approaches. Prefer someone with prior case management experience.
