Assertive Community Treatment (ACT) Team Lead

 

Coleman is seeking a Team Lead for our Assertive Community Treatment (ACT) in Portage County, Ohio.  ACT is an evidenced-based, shared-caseload, multi-disciplinary practice of delivering comprehensive community based behavioral health services to adults with certain serious and persistent mental illnesses.  Duties will include providing clinical supervision of ACT team members, clinical leadership, administrative/data management tasks, and some limited direct care and advocacy for consumer needs.  Some on-call responsibilities will be required.  Coleman is seeking a Team Lead for our Assertive Community Treatment (ACT) in Portage County, Ohio.  ACT is an evidenced-based, shared-caseload, multi-disciplinary practice of delivering comprehensive community based behavioral health services to adults with certain serious and persistent mental illnesses.  Duties will include providing clinical supervision of ACT team members, clinical leadership, administrative/data management tasks, and some limited direct care and advocacy for consumer needs.  Some on-call responsibilities will be required.

Must have LISW, LPCC, RN, or Psychology licensure and eligible to obtain an NPI number and be enrolled as a provider to bill Ohio Medicaid.  Must also be able to communicate effectively with consumer, families, multi-disciplinary staff, and partner agencies in addition to having at least 3 years’ experience and training in serving the SMI, SPMI, SUD and/or co-occurring disorders populations.  A valid Ohio driver’s license and clean driving record a must.  Experience in budgeting and/or program evaluation is preferred.  Salary based on experience and credentials.
